Start tracking before your next appointment — ovulation pain, one-sided pain, cycle day, severity, location, triggers. Whether you’re seeing a doctor, an acupuncturist, or a naturopath, tracking is something that helps to speed up getting clear answers.
If you have PCOS/PMOS and you’re trying to underst If you have PCOS/PMOS and you’re trying to understand your cycle—especially if you’re trying to conceive—basal body temperature can be a helpful tool.

After ovulation, progesterone usually causes your temperature to rise slightly and stay higher for at least three days. With PCOS, ovulation may occur later, so your lower-temperature phase may last longer and the rise may happen later than expected.

You may also see more than one LH rise in a cycle. If you get a positive LH test but your temperature stays in your usual low range, keep tracking rather than assuming ovulation definitely occurred. A later LH rise may be followed by a more meaningful temperature shift.

As an example, if your BBT remains around 36.2–36.5°C and then rises to 36.7–36.8°C and stays higher for at least three days, that pattern may suggest that ovulation likely occurred.

Also pay attention to other symptoms around the time of ovulation: changes in cervical mucus (clearer, wetter, more stretchy), bloating, breast tenderness, or mild cramping that feels different from your usual period cramps. These signs do not confirm ovulation on their own, but together with your BBT pattern they can give you a clearer picture of your cycle.

Keep in mind, especially with PCOS, that in some cycles your body may have attempted to ovulate but ovulation was not successful. This is why looking at the overall pattern—rather than one isolated sign—is so important.

There is no universal “low BBT” cutoff. Your own baseline and the sustained change from that baseline are more informative than one specific number.

If your OPK says positive but your temperature doesn’t rise, or you see more than one LH surge in a cycle, you’re not doing anything wrong. 

PCOS/PMOS can mean irregular or infrequent ovulation, and the hormonal patterns are often less predictable than a textbook cycle.
If you have PCOS or PMOS (polyendocrine metabolic If you have PCOS or PMOS (polyendocrine metabolic ovarian syndrome) and you’re trying to conceive, here’s something important to keep in mind: ovulation may happen later than you expect!

You may also see more than one rise or positive result on an LH test. With PCOS, LH can fluctuate or be elevated at baseline, so an early positive test doesn’t always confirm that ovulation has happened.

In some cycles, your body may make an attempt to ovulate, but the actual ovulation may happen later—so a second LH rise could follow.

This is why it can be helpful not to stop tracking after one positive test. Consider continuing to watch for other signs of ovulation, such as cervical mucus or a sustained basal body temperature rise.
